Job Description:
A company is looking for a Telephonic Care Navigator.
Key Responsibilities
Completes telephonic outreach to members and assists with
scheduling provider appointments
Educates members on gaps in care and links them to community
resources
Maintains confidentiality of health information and meets quality
and performance metrics
Required Qualifications
High School Diploma / GED
Must be 18 years of age or older
1+ years of experience in customer service, preferably in a call
center
1+ years of healthcare, insurance, or community outreach
experience
Proficient in Microsoft Office applications
