Recognized as a Top 100 Critical Access Hospital by the National Rural Health Association for eight consecutive years, KHC provides comprehensive, patient-centric care to the local community.Physical Therapist Requirements:EDUCATION AND EXPERIENCEEducation: Graduate of an APTA approved four year bachelors degree program, or foreign equivalent.Licensure: Current PT License through the AZ Board of Physical TherapyCertification: BLS and AED through the American Heart AssociationExperience: Recent experience working with an adult and geriatric populationPreferences:Education/experience serving people with physical disabilities is desirable.Work Requirements:The Incumbent shall have the ability to lift and transfer patients and equipment, have the ability to stand, walk, sit, bend, stoop, crouch, and communicate to provide therapeutic interventions.Essential Functions:If you are highly organized, proactive, and passionate about connecting talented individuals with meaningful roles, we would love to discuss this opportunity with you!Conduct a Physical Therapy evaluation or screen.Develop an individualized Plan of Care with functional goals.Provide appropriate therapeutic interventions.Evaluate patient response to treatment and modify plan as necessaryDocument the course of patient care including progress made and the continuing need for treatment.Provide patient and/or family education to help achieve goals.Adheres to KHI Attendance Policy.Communicates and demonstrates a professional image for patients, families, clients, coworkers, and others.Adhere to policies, rules, and regulations from KHI, Arizona State Board of Physical Therapy, and Medicare.Assists with orientation of new staff as requested.Meets KHI requirements for skills competency and safety.Participate in continuing education/professional development activities.Assists with the Physical Therapy and Physical Therapy Assistant student program.Participate in staff meetings, client relation and marketing activities.Documentation is complete and accurate, meeting all KHI, Arizona State Board of Physical Therapy, and Medicare regulatory requirements.Billing is completed by the end of each shift, accurate, meets reasonable and necessary requirements, and is justified in the documentation.Individual productivity meets department standards.Patient satisfaction will meet or exceed department standards.Additional InformationFor immediate consideration please call/Text: 657-643-3945by Jobble
