*Home Health Physical Therapist* *OPENINGS *: Per Diem

Mare Island Home Health Inc. currently provides services to the following cities in the following counties:
* Solano County
* Napa County
* Yolo County
* Contra Costa County

Initial Requirements:
* *Current license to practice in CA*
* *Current CPR certification*
* *Current CA driver's license plus proof of car insurance.*
* *Minimum one year clinical experience (two+ years preferred)*
* Bi/Multi-lingual skills are highly desired (e.g. Spanish, Russian, Tagalog, etc.)

Required experience:
* Physical Therapist: 1 year

*Responsibilities*
* Provide physical therapy services to patients in their homes, focusing on patient care and rehabilitation.
* Utilize knowledge of physiology, anatomy, and physical therapy techniques to create personalized treatment plans.
* Incorporate medical terminology into documentation and communication with healthcare professionals.
* Implement discharge planning strategies for smooth transitions post-treatment.
* Collaborate with interdisciplinary teams to ensure comprehensive care, especially in acute care settings.
* Adapt care plans to meet the unique needs of pediatric patients, emphasizing a holistic approach to therapy.

Job Types: Part-time, Per diem

Pay: $140,000.00 - $164,000.00 per year
